Note: There is a 24 hour waiting period to receive the card. Please do not expect to receive the card on the same day you ask for it. See the Visitor Services page for circulation policies and costs. Alumni may receive free Alumni Cards by visiting the DukeCard Office at 100 West Union Building. Please complete all elements of the form and click the submit button below. All fields are required.

I hereby request the privilege of borrowing books and agree to read and abide by the regulations below:

Loans are for 4 weeks and may be renewed twice.
Books must be returned immediately if recalled.
External borrowers may not recall books.
Overdue or lost book charges will cancel borrowing privileges.
Losing books is grounds for revocation of card priviliges.
Do not allow anyone else the privilege of using your card to check out the library's materials or use your card to check out books for other people.
